Student Emergency Assistance Fund (SEAF) Application Checklist
Please use this checklist as a guide for gathering the necessary information and submitting a complete Student Emergency Assistance Fund application.
After you submit your application, please monitor your Lynx Student Email for updates regarding your Student Emergency Assistance Fund (SEAF) application. Reply emails will have your Student Name on the Subject line and come from StudentAffairs@carrollcc.edu email address.
PERSONAL STATEMENT – Address ALL four topics in your statement. Brief, four to five sentences addressing ALL four prompts are sufficient.
- Describe your financial emergency in detail (e.g., what happened?). Examples: loss of job; reduction in income or work hours; limited income.
- Describe what you need to use the money for specifically (e.g., what for?). Examples: emergency housing, unexpected utility/electric bill, emergency room bill, unexpected car repair, or other circumstance.
- Indicate the emergency dollar $ amount you are requesting.
- Explain how your financial emergency is impacting your college education.
Examples: taking online classes and need electricity to charge devices; need additional books/supplies and reduction in work hours or limited income keeps me from affording books/supplies
SUPPORTING DOCUMENTATION – Upload a minimum of one document from each of the categories below.
IMPORTANT: The College does not automatically award the maximum dollar amount you request; rather, we determine the SEAF award based on the acceptable documentation you provided from Category 1 and the availability of SEAF funds at the time an application is approved.
- Document needs to include your name and relate to the information you provided in your personal statement.
- Examples of acceptable documentation:
- Invoice
- Cost estimates
- Past due utility bill statements (e.g., electric bill)
- Emergency Room Receipt(s)
- Examples of acceptable documentation:
- Employment Termination Notice
- Unemployment Letter
- Paystubs showing change in income (e.g., reduction of work hours)
